Addressing ‘Ozempic Face’: Why Even a Healthy BMI Doesn’t Always Mean a Healthy-Looking Face

Discover the truth about 'Ozempic face' and 'Mounface' – facial volume loss linked to weight loss medications like Ozempic and Mounjaro. In this expert blog from Fitzrovia Clinic, Dr Toni Burke explains why even those with a healthy BMI can experience facial hollowing, sagging skin, and premature ageing.

Introduction

Over the last year, we’ve seen a significant rise in patients visiting Fitzrovia Clinic with a very specific concern:
“I’ve lost weight and feel great — but why do I look so tired?”

Often, they’re using GLP-1 medications like Ozempic (semaglutide) or Mounjaro (tirzepatide). These have become household names, praised for their effectiveness in supporting weight loss and metabolic health.

But as clinicians, we’re also seeing the flip side: Facial hollowing, skin laxity, and premature ageing — even in people who are otherwise healthy, thriving, and happy with their weight.

It’s led to phrases like “Ozempic Face” and — our own affectionate term — “Mounface.”
Let’s explore what’s going on, and what you can do about it.

What Is “Ozempic Face” or “Mounface”?

Both refer to the facial changes that can occur following weight loss with GLP-1 receptor agonists. Even modest fat loss in the body can result in noticeable changes in the face, including:
– Hollowing of the cheeks or under-eye area
– A sharper or sometimes harsher-looking jawline
– Sagging around the lower face or jowls
– Thinner or looser skin
– An overall “tired” or less vibrant appearance

Many of these patients have a normal BMI (22–25) or have only lost a few kilograms. But when fat pads in the face shrink — especially around the cheeks, temples, and eyes — the change can be surprisingly dramatic.

Why Does This Happen?

Facial volume is supported by a combination of fat pads, bone structure, collagen, and skin elasticity. GLP-1 medications work by mimicking a hormone that suppresses appetite and slows digestion, leading to a substantial drop in calorie intake.

When weight loss happens too quickly — or over a prolonged period — the face doesn’t always have time to adapt. This is particularly pronounced in people over 35, when collagen production is already declining and bone density is slowly reducing.

Even with a healthy BMI, the skin can start to appear less supported and more vulnerable to fine lines, shadows, and sagging.

It’s Not Just About Weight Loss…

Many people are staying on these medications long-term — not just for obesity or diabetes, but for ongoing weight control, hormonal regulation, or appetite suppression.

Some patients are even microdosing, taking small amounts continuously to prevent rebound hunger or regain.

This means:
– The body may remain in a mildly catabolic state
– Fat pads in the face remain depleted
– Skin hydration and collagen may gradually decline over time

Even without dramatic weight loss, we’re seeing patients with long-term structural and textural changes to the skin — particularly in the face, neck, and chest.

Supporting Your Skin While on GLP-1s

You don’t necessarily need to stop the medication — many people experience life-changing benefits. But it does mean you need to be proactive about skin health — both from the inside and outside.

Nutritional Support:
– Don’t skip meals — even if you feel full
– Include high-quality protein daily to support skin and tissue repair
– Supplement with collagen-boosting nutrients (vitamin C, zinc, omega-3s)
– Stay hydrated
– Avoid excess alcohol and caffeine

Topical Support:
– Use barrier-strengthening skincare (ceramides, peptides, hyaluronic acid)
– Avoid overuse of actives (retinol, acids)
– SPF daily — always
